Clinical Case Manager - ACT
Full-time position: Assertive Community Treatment (ACT) is a client centered, recovery oriented, mental health service delivery system. Provides community based case management and psychosocial rehabilitation services for individuals with the most severe mental illness. Plans, provides, coordinates, and monitors therapeutic, rehabilitative, support and community integration services to assist individuals to integrate as independently and fully into the life of the community as possible. Works collaboratively as part of a multidisciplinary treatment team and must demonstrate excellent clinical judgment, decision making and safety consciousness to assure safety and appropriate treatment for consumers in the community. Qualifications: MA Degree in psychology, social work, psychosocial rehabilitation or related human services field with at least one year in a degree-related internship under the supervision of a psychiatrist, doctoral level psychologist, a certified/licensed mental health professional or person with equivalent credentials. Two years experience in community based psychosocial rehabilitation treatment or Case Management services preferred.
